AB - Transport and mining companies are vulnerable to a variety of hazards, and this paper offers a novel conceptual framework for organisational resilience assessment at different organisational levels. It is based on a safety climate and performances assessment in companies in the transport and mining sector. The framework contains factor and reliability analysis and continues by applying the SMART method from the perspective of resilience corners: anticipate, monitor, react and learn. The highest resilience coefficients at all organisational levels are obtained regarding safety awareness, safety training and safety communication areas, while the lowest values are observed for risk assessment and the organisational environment. The obtained results indicate that the ‘monitor’ resilience corner must be improved at all organisational levels, employees trained and procedures changed, since employees do not sufficiently use experience from previous events. The very high values at all organisational levels of the coefficients regarding the ‘anticipate’ and ‘learn’ resilience corners indicate organisational adaptation to changes through corrective activities, rather than planned preventive ones. All hierarchical levels have good resilience indicators, except for middle management, which expressed an adequate resilience index, due to the lower ratings of the safety training, organisational commitment, risk assessment and management support areas.

AB - There are rare studies carried regarding the assessment of the anthropometric convenience of transport and mining machinery. The first aim of this paper is to examine if there are differences between transport and mining machinery operators from the aspect of foot controls, the second aim is to compare the most common causes of downtime and failure and the third aim is to compare if there are differences in the field of injuries at work of both machinery types. Samples of 31 transport and 65 mining machinery operators were examined. Analysis is done by using descriptive statistics, the Kolmogorov test for normality, Mann–Whitney U test, and Pareto analysis. Comparison of operators’ height and weight, vibrations feeling through the foot controls and its easiness to be reached and used/controlled and injuries rates have not shown statistically significant differences found between transport and mining machinery. Pareto analysis on failures and stoppages of both types of machinery found completely different causes in the field of “vital few”. Anyhow, since statistically proved facts show that there are no differences regarding safety and human factors issues it could be indicated to designers that there is the possibility of applying the same innovative solutions to both types of mechanization in the field of foot controls.

AB - The paper presents the methodology of identification and
analysis of stoppages in mining machinery. Methodological steps are aimed at
isolating stoppages grouped by type of cause and summarized according to
criteria: frequency of occurrence and down time in minutes. In addition to the set
criteria, the paper also took into account the human factor, i.e. the operators of the
observed mining machines. Mining machinery operators` attitudes were surveyed
through a questionnaire covering several areas of occupational safety and health.
The experimental part of the research included 13 mining machines, and then, on
the example of a wheel loader, the key causes of down time were identified and the
attitudes of the operators of this machine were analyzed in terms of aspects of
occupational safety and health, because this mining machine had the longest down
time in the sample. In the final deliberations, it was concluded that the
organizational environment affects reduced safety and health at work, that is, that
the accelerated work rhythm leads to negligent use of the machine and absence
from safety procedures. Finally, research indicates that by work planning and
organizing that does not impose an accelerated work rhythm, safety and health at
work can be significantly improved.

AB - Risk management of engineering systems is of vital importance, but very often there
is no enough data to quantitatively evaluate risk and accordingly create risk management
strategy. Proper maintenance of excavators and bulldozers maximizes fuel efficiency and
reduces operating costs, as well as reduces equipment failure and enhances safety. Accordingly,
previous research opens avenue to analyze these two machinery types when working in mining
industry. This paper aims to create statistical comparisons on data collected about excavators’
and bulldozers’ as technological failure/stoppage, electrical failure/stoppage, mechanical
failure/stoppage, misuse, organizational failure/stoppage and external cause of
failure/stoppage. After performing descriptive statistics, hypothesis testing has not found
significant differences between technological failures/stoppages of excavators vs. bulldozers,
mechanical failures/stoppages between already mentioned machines or theirs hazard degrees
or total downtimes. Further research focus should be directed to the sample enlargement and
further analysis, which is expected to prove preliminary results.

AB - Objectives. The analysis of previous research shows that indicators of human factors have not been sufficiently integrated into the models for risk assessment of pressure equipment to date. Therefore, the goal of this article is the creation of a universal measurement scale to assess the current condition of the impacts of human factors on the risk of pressure equipment exploitation in factories and plants. Methods. A research instrument with nine constructs and 61 dimensions was designed and tested on a sample size of 268 companies, by reliability, exploratory and confirmatory factor analysis. Results. The final model contains exact quantification of nine constructs described by 27 influencing human factors on risk associated with pressure equipment exploitation. Conclusions. The most influencing construct is 'communication', whereas the construct 'organizational change management' is the least influencing. Also, applying Pareto analysis shows that the most important constructs identified are 'communication', 'safety and health at work', 'potentially hazardous materials and equipment', 'maintenance/inspection', 'human error' and 'trainings and competences of employees for crisis situations'. It is a recommendation for the process industry enterprises, where pressure equipment is in operation, to include the obtained model in the risk assessment processes they are using.

AB - Objectives. Previous research indicates that models for risk assessment of pressure equipment have not sufficiently or adequately integrated indicators of human factors and organizational factors. Methods. The goal of this article is to check the differences in attitudes of operators and managers engaged in the area of pressure equipment risk management. A questionnaire was designed and data were then collected from 253 operators and managers working with the same pressure equipment in Serbia. By applying a paired t test, differences in their attitudes were checked for issues described by the constructs of communication (COM), potentially hazardous materials and equipment (PHME), process safety (PS), safety and health at work (SHW), organizational change management (OCM), maintenance/inspection (MI) procedures, human error (HE), trainings and competences of employees for crisis situations (TCECS) and conducting research after accidents (CRAA). Results. It is statistically evident that the coincidence of attitudes accounted for less than 20% of questions. The highest coincidence is shown for PHME and TCECS, where one-half of the dimensions of the given constructs indicate that there are no statistically significant differences. Conclusions. The differences found could be used as an explanation for high-risk levels of pressure equipment exploitation and how they influence it.

abstract = "Objectives. The differences found could be used as an explanation for high-risk levels of pressure equipment exploitation and how they influence it.",

AB - As industrial systems represent a complex socio-technical system, it is necessary to analyse the impact of manager-operator-machine interaction on industrial safety, as categories of contextual factors. However, modern scientific literature indicates insufficient research on this topic. This paper has an aim to empirically analyse behavioural style and transport and mining machines operators’ attitudes in the safety climate context. Participants in this study were 28 crane’s and 65 mining machinery’s (excavators, bucket wheel excavators, bulldozers, loaders, graders, backhoe loaders, trenchers, dump trucks and scrapers) operators working in Serbian industrial companies. In the first step there is conducted descriptive statistics and followed by Kolmogorov’s and U* Mann-Whitney test to examine differences. Obtained results have shown that there were not statistically significant differences both between attitudes of operators on those two kinds of machines, namely, there are no statistically significant differences in terms of absenteeism due to poor working conditions, atmosphere of cooperation and togetherness among operators and the ways in which managers motivate and reward them. Between numbers of injuries at work happened by both machines’ types used there are no statistical differences, too. Also, further factor analysis has shown that examined operators’ and machines’ characteristics divide into two factors – one is focused to anthropometric characteristics presented by height and weight while another is focused on age of operator and machine and operator’s experience. It is recommended, in future research to enlarge sample, repeat statistical testing and analyse wider set of variables on examined matters in aim to discover pattern of anthropometric factors influence on behavioural factors.

AB - Although the biggest advancements could be expected from the mining industry as the
oldest one, it is still a major source of air pollution, there are more people hurt or injured than in
any other industry, while social conflicts around it are worldwide spread. The reason for the lack
of expected progress in the area is the most likely the fact that current research streams vary
barely focus to human and organizational factors, but technology-centered design dominates,
while risk management analysis are rarely applied. The probable reason is proper record keeping.
The aim of this paper is to present the data collected on downtimes of backhoe loaders, to propose
the methodology to be applied and show and analyze results obtained on its down time analysis
and risk management. This research analyzed the structure of downtime and consequences during
a period of one year of two backhoe loader working in mining site in Serbia. Downtimes were
classified as technological downtime, power / electricity downtime, mechanical downtime, misuse
due to abuse, organizational downtime and downtime due to external influences. In this research,
the frequencies of downtime were monitored by the stated categories of downtime. The
consequences of the identified delays were then assessed, and the risk calculated. By analyzing
the types of downtime by systems, it was determined that the priority action measures in order to
mitigate the risk of specific construction machines should be aimed at protecting workers during
technological operations of installing turbines, rear arms and the like. Although mechanical
downtime had the largest share in the risk calculation, the risk map for specific machines clearly
indicates that the technological procedures of assembly and disassembly are the most critical in
terms of risk.

AB - The paper presents a case study of dump trucks work efficiency testing as well as identification, classification analysis and risk assessment. The aim of the research was to determine the most common reason for unplanned stoppages in the observed dump trucks and to evaluate them from the aspect of risk after classifying the stoppages according to dumper systems. In the research, a semi-quantitative method was used for risk assessment, considering that the set criteria, the downtime and the frequency of stoppages, were taken based on the results of measurements over a period of 18 months, while a ten-level descriptive scale was used for the criterion of the level of danger. The results of the research indicated that there are certain systems on the observed machines that are of moderate risk and for which additional preventive maintenance measures are proposed in order to reduce the risk. The research results are visually presented three-dimensionally, given that three criteria were used for risk assessment.

AB - According to available literature, safety at work and productivity of cranes are a necessary and insufficiently researched topic. Both research directions are closely related because they depend on the number and type of stoppages that occur during operation of these construction machines.
The focus of research should be on identifying the stoppages that can have fatal consequences for employees on-site, or unintended consequences in terms of production loss, means of work, or environment. In the present research, an attempt is made to create a methodology for identification, analysis, assessment, classification, and calculation of stoppage risk
in cranes (classified by categories and causes). Criteria used for analysis, assessment, and calculation of stoppage risk: degree of stoppage danger, frequency of stoppage classified
according to stoppage categories/causes, and downtime according to the adopted classification. Experimental research on a sample of 1,091 stoppages recorded on several cranes
over a longer time interval has confirmed the hypothesis of generating a 3-criterion matrix for stoppage risk assessment sensitive enough to prioritize key crane stoppage in terms
of risk. Results of experimental research indicate mechanical downtime of 16.22 %, a frequency 45.35 % in downtime, and RPN number 80, at hazard level 10. Selectively, according to the cause of stoppage, the results indicate that the hoisting brake (6.48 % downtime at hazard level 10; 2.14 % stoppage frequency) and hoist gear/tooth breakage (5.29 % downtime at hazard level 10; 3.58 % stoppage frequency) are key mechanical causes of stoppages and crane risk generators. By mitigating the given risks, it is possible to improve safety and increase productivity.

AB - In this paper, problem of mining machinery efficiency is considered through impact of different types of downtimes. New methodology is suggested based on comparison impact of mechanical and technological mining machinery time in fault (M-t, T-t) and mechanical and technological mining machinery downtime frequencies (M-f, T-f) on machine work done (W). Benchmarking analysis is conducted by pairing variables M-t-W, T-t-W, M-f-W and T-f-W, and determining relationships between observed variables. By comparing the increment of the cumulative downtime curve with the cumulative curve of the machine's operating volume, it can be assessed whether the observed downtime develops a concave or convex character of the operating volume cumulative curve, in respect to significant losses. Results indicate that the relationship between cumulative downtime curve and cumulative work done curve can successfully serve as a parameter for evaluating the impact of specific downtime on the efficiency of the machine. A multi-criteria analysis, namely, an analysis of the criteria - frequency and time spent in downtime for each of the observed causes of downtime, clearly determines the critical pattern of downtime on the efficiency of the observed machine.

AB - BACKGROUND: Previous research has shown that the collection and analysis of crane operators' anthropometric characteristics is very important for operators' comfort, health and working ability and also from the aspect of performance, productivity and safety. OBJECTIVE: The first aim of this survey is to collect up-to-date anthropometric data of crane operators in Serbia and to collect those data for the first time in Libya. The second aim of this survey is to compare the collected data. The third aim is to model the interior space necessary to accommodate operators in the cabins they operate in Serbia and Libya. METHODS: Standing height, sitting height, lower leg length, upper leg length, shoulder width, hip breadth, arm length, foot length and weight data were collected of 83 Serbian and 50 Libyan crane operators. Descriptive statistics, correlation analysis and tests for differences between variables were conducted to examine differences between Serbian and Libyan crane operators and enable further modeling. The modeling of the cabin interior was done using both univariate and multivariate operators' models from both samples. RESULTS: There are only four common correlations between variables for both samples. The only measurement without statistical differences is shoulder width. Serbian crane operators have statistically higher values of almost all measurements in comparison to Libyans. The minimal cabin interior space dimensions are 1207 x 1080 x 1884 mm for Serbian operators and 1106 x 1040 x 1790 mm for Libyan operators when using univariate approach, while multivariate approach provides more precise and comfortable accommodation within 1327 x 1123 x 1926 mm for Serbian operators and 1203 x 1090 x 1830 mm for Libyan operators. Accordingly, our survey shows that percentile models include less than the intended population proportion in the design problem with few dimensions and depends on correlations among them. CONCLUSIONS: A number of problems are solved through modeling the crane operator workspace and it has been shown that cabins with different dimensions have to be offered to Serbian and Libyan markets.

AB - Previous research shows that organizational factors influence quality improvement programs, and when there is a fit, it is leading to better business performances. Accordingly, the purpose of this paper is an analysis of interdependence between organizational culture and quality improvement via testing the differences between dimensions and types of organizational cultures and applied procedures for quality improvement techniques on companies from 32 countries worldwide. Following detailed exploration of the available literature, data collection is conducted on 200 production enterprises in multinational supply chain. Upon this, further statistical examination is conducted by comparison of the companies in dependence of its locations - continents. Results show that there are significant differences on dimensions of organizational culture and applied quality improvement procedures depending of geographical location of companies. Accordingly, results of this paper prove that contextual approach promoted in ISO 9001:2015 has to be applied and organizations that operate in different countries and continents must decide how much to localize their organizational culture and related management practices to fit within the host country context.

AB - Previous research show that the safety aspects of cranes are not well researched. So, the first aim of this research is to collect and analyze data about crane related accidents that took a place between 1985 and 2018 in 71 countries worldwide with the aim to determine the occupational injuries and fatalities frequency in each country individually and to identify countries with the highest number of accidents resulting in one of two outcomes reported by using a Pareto analysis. The second aim of this paper is to identify the most significant groups of causes of crane accidents, based on data collected from a number of literature sources. Based on the results obtained, it is evident that Spain and Germany are the countries where almost half of injuries in crane accidents occur. The research has also shown that 80% of injuries occur in only 6 countries (Spain, Germany, France, Italy, Japan and Mexico), while 80% of fatal accidents occur in the following countries: Romania, China, Turkey, Bulgaria, Poland, Israel, Croatia and Spain. As the main causes of crane related accidents, mechanical equipment (including construction), improper use, assembly and disassembly and transport of the crane are recognized by using Pareto analysis, since 80% of consequences are results of those causes. The proposal for further research is a detailed analysis of the role of the human factor in the dominant causes of crane accidents.

AB - Although in recent years more and more attention is paid to risk-based maintenance
techniques and technical diagnostic methods are being implemented on an ever-increasing
scale, cranes still participate with as much as one third in all deaths in certain types of
industry. In line with that, the subject of this research is the verification of an instrument for
the investigation of accident situations of the crane transport system. After conducting a factor
analysis and reliability analysis, it was confirmed that the instrument with 8 dimensions of
different weight factors can be used as valid and reliable. Data were collected in 51 domestic
companies, where crane transport systems are used. The mean value of collected data shows
that the examination of accident situations of the installed crane transport system is at a very
low level in the domestic industry - 18.95 out of 75 points. The proposal of further research is
to link the obtained data with other variables that affect the operation of the crane transport
system, such as the commitment of management to risk management, training of maintainers
and operators, prescribed work procedures, process safety information, change management,
occupational safety and machine inspection procedures.

AB - Anthropometric data are essential for designers of products, while hand anthropometric measurements are of special importance due to the fact that grasp enables different manipulation tasks. Literature review shows that differences on anthropometric characteristics of the hand based on laterality in Serbian context have not been examined till now, so this study tested it on the sample containing 110 subjects - 23 left-handed and 87 right-handed. Hand anthropometric measurements include 30 anthropometric dimensions measured on each examined participant. Dimensions are taken by capturing the imprints of the subjects' outstretched hands. Collected data were subjected to descriptive statistics, t-test, Kolmogorov test and Mann-Withney U* tests. Also, 5th and 95th percentiles are calculated on all dimensions. Results show that there are no statistically significant differences based on laterality in Serbian context. Accordingly, hand tools and many other equipment, which are controlled by means of Serbian operator could be designed in the same manner both for workers whose dominant had is left and right. Also, percentiles values are calculated and should be taken into account in design processes. It is recommended, in future research to enlarge sample, repeat statistical testing and analyze hand grasp possible issues.

AB - This paper proposes innovation through implementation of a real-time computer-aided visual guidance system (VGS) which prevents crane accidents, caused by crane operator’s obstructed view, which can be the cause of collisions between load and obstacles and/or intrusions in the workspace. Proposed VGS allows predicting whether a dangerous event is going to occur and promptly alerts the crane-operator in order to let her/him taking corrective actions during the execution of crane-assisted shifting duties. VGS is equipped with interface which is designed to be simple in terms of the number of commands and easy to use in order to reduce the operators’ stress. Analyses of the economic feasibility of VGS application through cost-benefit framework follows. The payback period of invested funds for the purchase and installation of VGS is 1.5 years and installing and usage of VGS represents a very low risk investment.

Takođe, oba projekta pripadaju kategoriji projekata niskog rizika.
AB - Cranes' operations are extremely risky and hazardous tasks, as demonstrated in various surveys in the field of incidents' causes and effects. The most frequent cause of unsafe crane operation task execution is human error. Accordingly, innovation which enables cranes' safe operations, prevents "blind lifts" and helps crane operator to avoid potential error is urgent. Project SPRINCE has proposed computer-aided Visual Guidance System as a real time solution aimed to prevent crane accidents, caused by obstructed view. This paper analyses the economic feasibility of Visual Guidance System application in various scenarios in aim to connect human, technology and organization issues through cost-benefit framework. There are two proposed investment scenarios analysed: A) Producing and selling of crane cabins with real-time computer-aided Visual Guidance System and B) Usage (purchase) of crane cabins with real-time computer-aided Visual Guidance System. The economic appraisal has proved that the total economic benefit in both scenarios in the complete utilization period is several times higher than the buying price, while the internal rate of return is few times higher than the aggregate rate of interest paid. Also, the amount of time which takes to recover the cost of an investment is less than four years. Moreover, both scenarios belong to the category of innovation projects with very low risk.

AB - Although in recent years more and more attention is paid to risk-based maintenance techniques and technical diagnostic methods are being implemented on an ever-increasing scale, cranes still participate with as much as one third in all deaths in certain types of industry. In line with that, the subject of this research is the verification of an instrument for the investigation of accident situations of the crane transport system. After conducting a factor analysis and reliability analysis, it was confirmed that the instrument with 8 dimensions of different weight factors can be used as valid and reliable. Data were collected in 51 domestic companies, where crane transport systems are used. The mean value of collected data shows that the examination of accident situations of the installed crane transport system is at a very low level in the domestic industry - 18.95 out of 75 points. The proposal of further research is to link the obtained data with other variables that affect the operation of the crane transport system, such as the commitment of management to risk management, training of maintainers and operators, prescribed work procedures, process safety information, change management, occupational safety and machine inspection procedures.

AB - The collection and analysis of anthropometric characteristics for various passenger car user populations is a very important predecessor of the design process. The first aim of this survey is to collect up-to-date anthropometric data for male passenger car drivers in Serbia and to collect those data for the first time in Libya. The second aim of this survey is to compare the collected data and enable further modeling using both univariate and multivariate operators' models from both samples. Anthropometric measurements on standing height, sitting height, lower leg length, upper leg length, shoulder width, hip breadth, arm length, and foot length were collected for 921 Serbian and 300 Libyan male drivers. Results show that, for Serbian male drivers, a correlation between variables exists in 67.85% cases, while for Libyan male drivers, a correlation between variables exists in 39.28% cases. The comparison of mean values between variables for Serbian and Libyan male drivers showed that, beside shoulder width, all others have significantly higher values for Serbians than for Libyikans. Consequently, there are different interior space dimensions of the vehicle obtained: for Serbian drivers, interior space dimensions amounted to 1500 x 561 x 1230 mm and for Libyan drivers amounted to1400 x 591 x 1155 mm which are useful to passenger car designers in eliminating existing anthropometric inconsistencies.

AB - Gap analysis represents a tool for raising the level of performances of products, processes and enterprise organization which is rarely used in risk management. This paper proposes the joint application of Gap and Pareto analysis, in aim to mitigate possible risks in production processes. It is based on the facts that key points in the production process indicate some serious oversights (gaps), characterized as errors, which can grow into risky elements that disturb the manufacturing process and final transmitter assembly. In this paper, finalizing and assembling pressure transmitter elements (modules), created by a domestic manufacturer, served as an example for the Gap analysis. Each electronic transmitter is consisted of three modules: measurement cell, mechanical coupling fixture and enclosure containing the electronics and the terminal block box. Through the implementation and assembly of these modules errors (or elements of potential risks) have been identified. Later on, using the Pareto chart, it has been seen that 80% of errors made during the transmitter manufacturing process have occurred while implementing the first and the third transmitter module. Also, by analyzing the collected gaps, it has been concluded that the critical ones happen while using the existing technology and engaging workforce. In order to eliminate the above-mentioned errors, this paper decidedly presents the Gap analysis steps which should be followed, so the transmitter manufacturing process would be improved in terms of quality. Similar methodology could be applied to other products and processes.
