The safety climate, hierarchical levels and resilience assessment in transport and mining companies

AB  - Transport and mining companies are vulnerable to a variety of hazards, and this paper offers a novel conceptual framework for organisational resilience assessment at different organisational levels. It is based on a safety climate and performances assessment in companies in the transport and mining sector. The framework contains factor and reliability analysis and continues by applying the SMART method from the perspective of resilience corners: anticipate, monitor, react and learn. The highest resilience coefficients at all organisational levels are obtained regarding safety awareness, safety training and safety communication areas, while the lowest values are observed for risk assessment and the organisational environment. The obtained results indicate that the ‘monitor’ resilience corner must be improved at all organisational levels, employees trained and procedures changed, since employees do not sufficiently use experience from previous events. The very high values at all organisational levels of the coefficients regarding the ‘anticipate’ and ‘learn’ resilience corners indicate organisational adaptation to changes through corrective activities, rather than planned preventive ones. All hierarchical levels have good resilience indicators, except for middle management, which expressed an adequate resilience index, due to the lower ratings of the safety training, organisational commitment, risk assessment and management support areas.

AB  - For the production planning process to be successful, an enterprise's internal business processes
must be effective and efficient, while the DMAIC cycle is important aspect of practice-based continuous
improvement. In accordance to those facts, this preliminary research includes the first part of the DMAIC
methodology - “define“ and “measure“ tools, applied to a case study in automotive industry, with the aim of
improving production planning processes. Analysis in “define“ phase starts with SIPOC, continues with
calculation of critical indicators in “measure“ phase and is followed by Pareto charts. Research shows that
the existing method of manual collection of production data is not precise enough, and during two months
observation period, between 14 and 52 percent of planned production time on 9 observed machines, passed
as unnoticed downtime. Further data analysis showed that this time is 1.5 to 9 times higher than the total
reported downtime on individual machines. Results show that the further development of data collection
tools is crucial, and the recommendation is to move in the direction of automation of that process in order
to make the most of available technical resources, in “improve“ phase and to “control“ it by statistical
comparison of previous and new state indicators

Transport and Mining Machinery Foot Controls: Safety and Human Factors View

AB  - There are rare studies carried regarding the assessment of the anthropometric convenience of transport and mining machinery. The first aim of this paper is to examine if there are differences between transport and mining machinery operators from the aspect of foot controls, the second aim is to compare the most common causes of downtime and failure and the third aim is to compare if there are differences in the field of injuries at work of both machinery types. Samples of 31 transport and 65 mining machinery operators were examined. Analysis is done by using descriptive statistics, the Kolmogorov test for normality, Mann–Whitney U test, and Pareto analysis. Comparison of operators’ height and weight, vibrations feeling through the foot controls and its easiness to be reached and used/controlled and injuries rates have not shown statistically significant differences found between transport and mining machinery. Pareto analysis on failures and stoppages of both types of machinery found completely different causes in the field of “vital few”. Anyhow, since statistically proved facts show that there are no differences regarding safety and human factors issues it could be indicated to designers that there is the possibility of applying the same innovative solutions to both types of mechanization in the field of foot controls.

Methodology for Analysing Risk Factor on Surface Top Hammer Drill Rig

AB  - Two-level risk mapping and assessment methodology was proposed with several criteria for risk prioritization (frequency of occurrence, downtime, level of danger, type of stoppage, cause of downtime, risk number). In a period of one year all stoppages, both planned and unplanned, were monitored and later on classified by type into: technological, mechanical, electrical, stoppages caused by the machine operator, external stoppages, which include stoppages of machine inactivity due to weather conditions and the like. The methodology proposed is oriented towards the calculation of risks to people, property, etc., which arise from various types of downtime. Then, based on the recording of data on stoppages of the observed machine, the structure of the share of individual stoppages was analyzed and appropriate measures to prevent
incidents/accidents were recommended. The main causes of stoppages are isolated based on the frequency of occurrence, the percentage of the downtime duration in the total time spent and the level of danger. The primary cause of stoppages according to the mentioned criteria is the rupture of the hose in the observed machine with percentage participation in the duration of mechanical stoppages of 25.17%, with a frequency of 25% and a degree of danger of 6, while repairing the rod clamp is the second causal factor singled out as important from the risk aspect. Thanks to this, it is possible to plan maintenance activities and manage risks in an adequate way. The proposal for further research is further performance prediction and the determination of optimum operating parameters for different working conditions.

AB  - For mining processes to operate safely, information about safety is crucial. Numerous authors pay attention to fostering a good safety climate without paying attention to safety communication across organizational structure, which motivates this survey. Accordingly, this paper aims to check the hypothesis whether there is a difference between the attitudes of managers, operators, and auxiliary workers in the mining industry regarding safety communication, since differences on their attitudes about safety communication issues have potential to cause safety performance. A survey of 123 respondents working in different positions in Serbian mining companies, which have evaluated the importance of communication as well as the current state regarding the quality of communication in their organizations, was conducted. After evaluating the data and conducting descriptive statistics, followed by the Mann-Whitney test, it was determined that there are no statistically significant differences in the attitudes of managers, operators, and support staff toward safety communication. This indicates that issues with an organization's safety performance are not brought on by the differences on safety attitudes at different hierarchical levels. Proposal for the future research is to examine other possible causes, such as cognitive biases in risk perception, safety training and education and similar, and to use larger sample that may confirm our hypothesis.

abstract = "For mining processes to operate safely, information about safety is crucial. Numerous authors pay attention to fostering a good safety climate without paying attention to safety communication across organizational structure, which motivates this survey. Accordingly, this paper aims to check the hypothesis whether there is a difference between the attitudes of managers, operators, and auxiliary workers in the mining industry regarding safety communication, since differences on their attitudes about safety communication issues have potential to cause safety performance. A survey of 123 respondents working in different positions in Serbian mining companies, which have evaluated the importance of communication as well as the current state regarding the quality of communication in their organizations, was conducted. After evaluating the data and conducting descriptive statistics, followed by the Mann-Whitney test, it was determined that there are no statistically significant differences in the attitudes of managers, operators, and support staff toward safety communication. This indicates that issues with an organization's safety performance are not brought on by the differences on safety attitudes at different hierarchical levels. Proposal for the future research is to examine other possible causes, such as cognitive biases in risk perception, safety training and education and similar, and to use larger sample that may confirm our hypothesis.",

AB  - The process of assembling micro components requires careful planning where precise and reliable data
collecting is necessary due to a short production cycle time. After determining the shortages of the
current data monitoring system, in this paper, by using the „Improve“ and „Control“ phases of DMAIC
cycle, the development and later testing of a new tool for automatic capture and analysis of data obtained
from the machine was done. SWOT analysis was used to establish the production site's and current
manufacturing execution system strengths and weaknesses, opportunities and threats, which were then
utilized to create the newly developed solution accurate and feasible. A tool calibration data was
obtained on 9 machines in automotive company's micro component assembly unit. To confirm the
accuracy of the data, a RunAtRate test was done on the machines, and by using hypothesis testing, the
initial reliability of the new solution was determined. Further testing and comparison revealed that the
newly proposed tool recorded 73-80% greater downtime and 40-50% more defective parts than the
previously installed system. Accordingly, the usefulness of the proposed Lean Six Sigma tool is
confirmed, and for periodic accuracy checks and further improve.ment, it is necessary to apply the PDCA
cycle.

AB  - Previous research shows that Kaizen's benefits are multiple and evident,
but its practices in the supply chain have been sufficiently examined now.
Conversely, we are witnessing numerous issues in contemporary global
supply networks. In this survey, after conducting a literature review, three
research questions regarding Kaizen modes of usage were formulated and
tested on the sample of 195 enterprises that are part of the global supply
chain, located in 31 countries, and active in two different types of
industries – aircraft, and transportation. A combined approach containing
descriptive statistics, reliability, factor analysis, and statistical hypothesis
testing by Kruskal-Wallis one-way ANOVA and Mann-Whitney U tests
were used. Results show significant differences between Kaizen practices
applied in countries such as Italy, the United Kingdom, Canada, the USA,
Japan, and China, where national and corporate cultures differ. Kaizen
implementation significantly differs between companies operating in the
aircraft and transportation sectors, which is unsurprising since aircraft
industry has a higher formalization level. The goal to determine the
differences in Kaizen practices around the globe was fulfilled since
statistically significant differences indicate the importance of the
contextual factors and connect adverse and Kaizen events.

Validating Measurement Structure of Checklist for Evaluating Ergonomics Risks in Heavy Mobile Machinery Cabs

AB  - Heavy mobile machinery cabs and their equipment are still not well adjusted to operators, so it is not surprising that we are still witnessing huge consequences of accidents at sites where they operate. The checklist with 39 questions, based on the previous research, is formed, and its’ measurement structure has been tested on the sample of 102 transport, construction, and mining machines, including cranes, excavators, bucket wheel excavators, bulldozers, loaders, graders, backhoe loaders, trenchers, dump trucks, and scrapers by correlation analysis, Cronbach’s alpha, Spearman-Brown and Kendall’s W coefficient, exploratory factor analysis, and confirmatory factor analysis. The results validate the measurement structure of a checklist with only 17 items and five constructs. The results show that special attention should be put to the design of armrests and working conditions/exhaust gases, which are negatively correlated to cab interior space and task visibility. All other correlations between seat characteristics, the characteristics of armrests, whole-body vibration influences, reaching commands, the characteristics of cab interior space and environments, and interpersonal relationships are positive, which means that improvements to one area lead to improvements in another. Accordingly, the proposed model should be used for the fast, efficient, and cost-effective evaluation of ergonomics risks and as a guideline for further cab design improvements.

AB  - A company’s development performance and growth may be impacted by a wide range
of different factors, which unquestionably affect number of the employees in the loop. Taking into
account all influencing factors, companies would benefit if have possibility to predict the degree of
change in the number of employees in future period in order to adjust their internal strategy or to
make appropriate decisions that enable the survival and progress of the company in the market. The
aim of this research is to predict the change in number of employees based on current state of
contingency and quality management factors, using information obtained from a survey of 67
different companies from Serbia. In the first part of the research, a correlation analysis is used with
the aim to identify the specific contingency and quality management factors that are most closely
associated to the subject of interest, which is, in this case, degree of change in the number of
employes. The second part of the research involves feedforward neural network training for
prediction of the degree of change in number of employees based on feature extraction of main
factors. The training accuracy that proposed network achieved is 77.36%, while testing accuracy
amounts 71.43%.

AB  - The paper presents the methodology of identification and
analysis of stoppages in mining machinery. Methodological steps are aimed at
isolating stoppages grouped by type of cause and summarized according to
criteria: frequency of occurrence and down time in minutes. In addition to the set
criteria, the paper also took into account the human factor, i.e. the operators of the
observed mining machines. Mining machinery operators` attitudes were surveyed
through a questionnaire covering several areas of occupational safety and health.
The experimental part of the research included 13 mining machines, and then, on
the example of a wheel loader, the key causes of down time were identified and the
attitudes of the operators of this machine were analyzed in terms of aspects of
occupational safety and health, because this mining machine had the longest down
time in the sample. In the final deliberations, it was concluded that the
organizational environment affects reduced safety and health at work, that is, that
the accelerated work rhythm leads to negligent use of the machine and absence
from safety procedures. Finally, research indicates that by work planning and
organizing that does not impose an accelerated work rhythm, safety and health at
work can be significantly improved.

AB  - Risk management of engineering systems is of vital importance, but very often there
is no enough data to quantitatively evaluate risk and accordingly create risk management
strategy. Proper maintenance of excavators and bulldozers maximizes fuel efficiency and
reduces operating costs, as well as reduces equipment failure and enhances safety. Accordingly,
previous research opens avenue to analyze these two machinery types when working in mining
industry. This paper aims to create statistical comparisons on data collected about excavators’
and bulldozers’ as technological failure/stoppage, electrical failure/stoppage, mechanical
failure/stoppage, misuse, organizational failure/stoppage and external cause of
failure/stoppage. After performing descriptive statistics, hypothesis testing has not found
significant differences between technological failures/stoppages of excavators vs. bulldozers,
mechanical failures/stoppages between already mentioned machines or theirs hazard degrees
or total downtimes. Further research focus should be directed to the sample enlargement and
further analysis, which is expected to prove preliminary results.

AB  - Mining industry has a vital role in economy every country. That is the reason why is necessary to detect, remove or mitigate and prevent risk failures of mining machinery. In order to achieve that the first step is to identify and compare downtimes/failures and estimated risks. In this paper it
was conducted the preliminary research that included 348 mining machines divided in 7 different types. The aim of the research was to determinate is there statistically significant difference between different types of mining machines in two criteria: level of danger of consequences of downtime/failure and estimated failure risk by using Kruskal - Wallis H Test. The results indicate
that when it comes to level of danger of consequences there is no statistically proof that there is difference between different types of the machines. In case of estimated failure risk, there is statistically significance between some types of mining machines.

AB  - U radu je prikazan model analize rizika rada dampera sa ciljem da se utvrdi najčešći razlog
neplaniranih zastoja kao i da se nakon klasifikacije zastoja po sistemima izvrši njihova ocena sa aspekta
rizika. Postavljeni kriterijumi za analizu rizika su: vreme provedeno u zastoju, frekvencija zastoja i nivo
opasnosti. Rezultati istraživanja ukazali su na uzročnike visokog rizika na posmatranim mašinama, na
osnovu kojih se mogu definisati prioritetne mere preventivnog održavanja u cilju smanjenja rizika.
Nadalje, u istraživanju prevencije rizika rada transportnih i rudarskih mašina razvijen je onlajn
kontekstualni i adaptivni ekspertni sistem HAMRISK i izvršeno ispitivanje prednosti i nedostataka
razvijenog softverskog rešenja u praksi.

AB  - Previous research points to the need for the development of a decision-making system in the field of adaptive and contextual risk management of operation and maintenance of transport and mining machines, which ensures the optimal choice of maintenance strategy and consequently greater safety and health at work, reduction of direct and indirect costs of incidents, better planning and greater productivity. Accordingly, this research work refers to the analysis of data obtained by surveying respondents - users of the HAMRISK expert system for risk prevention in transport and mining machines (cranes, excavators, bulldozers, excavators, loaders, etc.).In order to analyze data, a hierarchical structure of criteria with defined importance coefficients was generated. The model of the hierarchical structure of survey questions, taken into account with the estimated coefficients of importance, ensured a unique decision-making model. The structural hierarchical model of decision-making gives a more detailed insight into the analysis of the survey questionnaires for the assessment of the HAMRSIK expert system for risk prevention in transport and mining machines, and presented directions for further development and improvement of the first variant of the software solution. The obtained results were also used in order to design marketing activities for the promotion of the HAMRISK expert system.

AB  - In order to be competitive in the market, many companies must, besides product quality, constantly monitor the efficiency of their organization and its employees. Factor analysis can reduce the number of significant areas, by grouping some of them into influential contingent factors that help in monitoring of the situation. Also, for good and reliable planning, it is important to know how the areas are correlated with each other, to which correlation analysis contributes. This paper aims to analyze the data collected from the survey, conducted on a sample of 56 companies operating in Serbia, and to find influential factors. Descriptive statistics and data reliability were first performed and after that by factor analysis the data were divided into groups of influencing factors and then the correlation between the data was examined.  The obtained results indicate that it is possible to reduce 20 examined traits to 5 influential factors, and the correlation analysis shows the importance of  employee training on level of innovation in processes and products and faster and appropriate reaction to solving and preventive problems.

AB  - Objectives. Previous research indicates that models for risk assessment of pressure equipment have not sufficiently or adequately integrated indicators of human factors and organizational factors. Methods. The goal of this article is to check the differences in attitudes of operators and managers engaged in the area of pressure equipment risk management. A questionnaire was designed and data were then collected from 253 operators and managers working with the same pressure equipment in Serbia. By applying a paired t test, differences in their attitudes were checked for issues described by the constructs of communication (COM), potentially hazardous materials and equipment (PHME), process safety (PS), safety and health at work (SHW), organizational change management (OCM), maintenance/inspection (MI) procedures, human error (HE), trainings and competences of employees for crisis situations (TCECS) and conducting research after accidents (CRAA). Results. It is statistically evident that the coincidence of attitudes accounted for less than 20% of questions. The goal of this article is to check the differences in attitudes of operators and managers engaged in the area of pressure equipment risk management. AB  - As industrial systems represent a complex socio-technical system, it is necessary to analyse the impact of manager-operator-machine interaction on industrial safety, as categories of contextual factors. However, modern scientific literature indicates insufficient research on this topic. This paper has an aim to empirically analyse behavioural style and transport and mining machines operators’ attitudes in the safety climate context. Participants in this study were 28 crane’s and 65 mining machinery’s (excavators, bucket wheel excavators, bulldozers, loaders, graders, backhoe loaders, trenchers, dump trucks and scrapers) operators working in Serbian industrial companies. In the first step there is conducted descriptive statistics and followed by Kolmogorov’s and U* Mann-Whitney test to examine differences. Obtained results have shown that there were not statistically significant differences both between attitudes of operators on those two kinds of machines, namely, there are no statistically significant differences in terms of absenteeism due to poor working conditions, atmosphere of cooperation and togetherness among operators and the ways in which managers motivate and reward them. Between numbers of injuries at work happened by both machines’ types used there are no statistical differences, too. Also, further factor analysis has shown that examined operators’ and machines’ characteristics divide into two factors – one is focused to anthropometric characteristics presented by height and weight while another is focused on age of operator and machine and operator’s experience. It is recommended, in future research to enlarge sample, repeat statistical testing and analyse wider set of variables on examined matters in aim to discover pattern of anthropometric factors influence on behavioural factors.

oldest one, it is still a major source of air pollution, there are more people hurt or injured than in
any other industry, while social conflicts around it are worldwide spread. The reason for the lack
of expected progress in the area is the most likely the fact that current research streams vary
barely focus to human and organizational factors, but technology-centered design dominates,
while risk management analysis are rarely applied. The probable reason is proper record keeping.
The aim of this paper is to present the data collected on downtimes of backhoe loaders, to propose
the methodology to be applied and show and analyze results obtained on its down time analysis
and risk management. This research analyzed the structure of downtime and consequences during
a period of one year of two backhoe loader working in mining site in Serbia. Downtimes were
classified as technological downtime, power / electricity downtime, mechanical downtime, misuse
due to abuse, organizational downtime and downtime due to external influences. In this research,
the frequencies of downtime were monitored by the stated categories of downtime. The
consequences of the identified delays were then assessed, and the risk calculated. By analyzing
the types of downtime by systems, it was determined that the priority action measures in order to
mitigate the risk of specific construction machines should be aimed at protecting workers during
technological operations of installing turbines, rear arms and the like. Although mechanical
downtime had the largest share in the risk calculation, the risk map for specific machines clearly
indicates that the technological procedures of assembly and disassembly are the most critical in
terms of risk.

AB  - Mining is considered one of the most risky industrial operations. Development and application of risk assessment methods has the greatest impact on improvements in terms of safety in mining equipment. Risk is analyzed in many aspects i.e. human factor, environmental impact and operational aspects. Analyzing the mining machinery is a way to determine which ones have the greatest tendency to fail. The aim of this research is to compare data on failures obtained from 348 mining machines. All machines are arranged in 7 groups, namely 50 excavators, 50 bulldozers, 48 drills, 50 dumpers, 50 backhoe loaders, 50 bucket wheel excavators and 50 loaders. In order to compare the data, it is necessary to first determine the distribution under which the data belong. That is achieved by using Kolmogorov-Smirnov test. After that the Kruskal-Wallis test was used to determine whether there is a relationship between the machines, viewed from the aspects of failure duration and failure frequency.

AB  - The paper presents a case study of dump trucks work efficiency testing as well as identification, classification analysis and risk assessment. The aim of the research was to determine the most common reason for unplanned stoppages in the observed dump trucks and to evaluate them from the aspect of risk after classifying the stoppages according to dumper systems. In the research, a semi-quantitative method was used for risk assessment, considering that the set criteria, the downtime and the frequency of stoppages, were taken based on the results of measurements over a period of 18 months, while a ten-level descriptive scale was used for the criterion of the level of danger. The results of the research indicated that there are certain systems on the observed machines that are of moderate risk and for which additional preventive maintenance measures are proposed in order to reduce the risk. The research results are visually presented three-dimensionally, given that three criteria were used for risk assessment.

mechanical downtime, misuse due to abuse, organizational downtime and downtime due to external influences.
In this research, the frequencies of downtime were monitored by the stated categories of downtime. The
consequences of the identified delays were then assessed, and the risk calculated.",

AB  - Mining is considered one of the most risky industrial operations. Development and application of risk assessment methods has the greatest impact on improvements in terms of safety in mining equipment. Risk is analyzed in many aspects i.e. human factor, environmental impact and operational aspects. Analyzing the mining machinery is a way to determine which ones have the greatest tendency to fail. The aim of this research is to compare data on failures obtained from 348 mining machines. All machines are arranged in 7 groups, namely 50 excavators, 50 bulldozers, 48 drills, 50 dumpers, 50 backhoe loaders, 50 bucket wheel excavators and 50 loaders. In order to compare the data, it is necessary to first determine the distribution under which the data belong. That is achieved by using Kolmogorov-Smirnov test. After that the Kruskal-Wallis test was used to determine whether there is a relationship between the machines, viewed from the aspects of failure duration and failure frequency.

AB  - The subject research is aimed at pointing out the methodology of identification, classification and risk analysis of mining machines. Bulldozers as heavy  mobile machines are very useful in mining processes as auxilary machines
which generate up to 20% of the total mining costs and cause numerous injections and fatalities. Till now, there have been numerous attempts to understand underlying causes of injury incidents on mining equipment, but available studies very rarely analyze and do not systematically identify, quantify and evaluate risks related to bulldozers. The risk calculation was realized by applying the semiquantitative method in risk assessment. The experimental part of the research was conducted on a mining machine, bulldozer Komatus K155AX, for which risk mapping was performed and conclusions on the most significant stoppages from the risk aspect were made. The results of the research indicated that there were no high and moderate risk stoppages in the observed machine. The most significant
stoppages were identified for danger level 6 mechanical types of stoppages. It is also significant that in the observed time period in which the work of the mining machine was recorded and monitored, no organizational stoppages or stoppages due to human factors were recorded, which indicates the efficiency of the maintenance function.

AB  - Research related to risk management of failures and stoppages in the  operation  of bulldozer is  very  limited,  so  this topic  needs  more  attention. This  study  aims  to  identify  all  possible  causes  for  bulldozer failures  and stoppages before highlighting the most significant ones. Descriptive statistics are  provided,  followed  by  a  scatterplot  for  the  dissipation  diagram  between hazard rate and bulldozer failure and stoppages, and histograms of hazard rate and failures and stoppages for bulldozers. Then, for three categories, a Pareto analysis  was  performed on failures  and  stoppages frequency, time  spent  in failures and  stoppages,  and failures  and  stoppages risk level.  The  Pareto analysis  revealed  that  the  most important  failures  and  stoppages in  all  three categories were caused by heating repair, oil change, bulldozer cleaning, tonsil adjustment, filter replacement, hose replacement and screw replacement. The step towards “maintenance free factories” should include planning of identified activities in work breaks as preventive rather than corrective activities
